Advance Directives

An advance directive makes your voice heard when you can no longer communicate. It tells your doctor and family members what kind of healthcare you would like to receive if you become unable to participate in making medical decisions for yourself. Having written instructions can help reduce confusion or disagreement. It is important that you discuss your preferences with your family and physician so that they will be aware of your wishes.

Advance directives include:

• Living Will: This document allows you to establish, in advance, the type of medical care you would want to receive if you were unable to make medical decisions for yourself. Also, a living will allows you to specify your wishes regarding anatomical gifts (organ and tissue donation).
• Health Care Power of Attorney (POA): This is a legal document that designates an individual to make medical decisions for you if you're unable to do so.
